2018-2019 Field Employees Benefits

Excel Home Care Care values the contributions of our Field Employees. In appreciation of your dedicated service, we are pleased to offer a variety of affordable benefit plans. Please carefully review this enrollment guide so you understand the benefits being provided and can make the right choices for you and your family.

What you Need to know…

We will continue offering the same coverage options as last year and rates for all benefits except for the Minimum Essential Coverage will remain the same for 2018. The Minimum Essential Coverage (MEC) rates have been reduced for 2018 and the plan covers all preventive services required by the Affordable Care Act (ACA) and satisfies the ACA’s Individual Mandate so you may not be assessed a penalty on
your taxes while enrolled in the MEC plan.

2018-2019 Field Employees Benefits



Current field employees that have satisfied the 60-day waiting period and work at least 20 hours per week are eligible for benefits now.
New or rehired field employees working at least 20 hours per week are eligible for benefits after completing the 60-day waiting period.
You can enroll between your 30th and 60th day with coverage becoming effective the 1st of the month following 60 days of employment.

  • Field employees working 20 to 30 hours per week are eligible for all benefits EXCEPT the Minimum Essential Coverage (MEC) plan
  • The Minimum Essential Coverage (MEC) plan is only available to Field employees that work 30 or more hours per week and are not
    covered by the Illinois Union Health Benefit Fund program.


You can only elect or make changes to coverage during open enrollment or during your 30/60 day window upon hire.

All elections made during open enrollment remain in effect for the entire plan year unless you have a qualified change in status.

The 2018 plan year begins on July 1, 2018 and ends on June 30, 2019. The next open enrollment will be held in June 2019.

Your Coverage Choices

• Minimum Essential Coverage (MEC): To be eligible you must work 30 or more hours per week and are not covered by the Illinois Union Health Benefit Fund program

• Limited Benefit Medical: Two benefit plans available

• Dental Benefit Plan

• Vision Benefit Plan

• Short-term Disability: Two benefit plans available

• Group Voluntary Accident Plan

During Open Enrollment you have the option to…

1) Enroll yourself and your dependents in benefits

2) Change or cancel your current benefits
      – Your current benefits will continue if you do not cancel them

If you want to continue your current coverage without any changes nothing further is required.
